Where to get malaria pills...

You can get underground missions (for malaria medication) at locations other than the Church in Pala and the clinic in Port Selao.  Once you unlock a underground house, you will see an icon at that location when you need malaria pills.  For instance, starting at the following location south of the Dogon Village:

The guy in the main room assigns you the mission

Once you arrive at the location, dispatch the guards and enter the building:

... to exchange the passports you have for the malaria pills.

The more underground houses you unlock, the easier it becomes to be close to a position on the map that will give you the underground missions.
